About Us

Victoria Costello, Administrator
Victoria was born in Sheffield, England and came to Texas at an early age.  She was raised in Refugio, a small town in South Texas.  After graduating from Refugio High School, she attended Texas A & I University in Kingsville where she received a Bachelor of Science Degree in Foods and Nutrition - Clinical Concentration.
Victoria began her career in long term care as a Dietary Consultant for skilled nursing facilities, and after moving to the Cedar Creek Lake area became the corporate dietitian for a Tyler based nursing home company.  In 1981 she received a Nursing Facility Administrator's license and transitioned into the role of administrator.  Since that time Victoria has primarily worked in the East Texas area.

When asked why she chose long term care as her career she replied "when I realized I would probably be working most of my life, I wanted to choose something that would give meaning to the time I would have to spend away from my family.  I knew that taking care of people who could no longer take care of themselves would be such a career.  I have never regretted my choice".

Allison Boggus, LVN, Assistant Director of Nursing
Mrs. Boggus is one of the Assistant Directors of Nursing at Autumn Leaves. She was born in Henderson and has lived in Texas all of her life. She attended nursing school at Blinn College in Brenham, TX and has been an LVN since 1999. Mrs. Boggus enjoys managing the Rehab hall, getting to see residents recover from serious injury or illness, and then return to the community.

Marsha Taylor, LVN, Admissions Nurse  Mrs. Taylor is the Admissions Nurse for Autumn Leaves Nursing & Rehab.  She has been on staff since July 2009. She graduated from Panola College, and has been practicing as an Licensed Vocational Nurse for 14 years. Matt Elmore, Environmental Director  I began my employment at Autumn Leaves in March, 2011. I work each day to ensure our building is safe and a comfortable environment for our residents and staff.

Vanessa Odom, Receptionist

Angels in the Kitchen Autumn Leaves employs some wonderful ladies who prepare healthy, delicious meals for our residents!

Misty Villescas, Activity Director
I have worked in the health care field for over 6 years, specializing in special needs for adults. I received my activity director certification from Kilgore College in November,2013. I love having the opportunity to interact with the residents here and to be able to provide them with fun activities and outings! Their smiles and hugs make my days!

Glenda Gatewood, Medical Records
Although Glenda is the newest member to the Autumn Leaves team, she is not new to the medical field.  She began her career in the medical field over thirty years ago. Besides having a passion for healthcare and a bright smile every day she is her church secretary and a mother of two. She enjoys doing the best she can do in everything she does!


Janis Cagle, Business Office Manager/HR

I have been working in long term care for 33 years, holding various positions caring for the elderly and disabled.  I began working on the corporate side of QRM in 2013, managing billing, then was asked to take a position with Autumn Leaves in March, 2018, to oversee the business office in-house as well.  Although not directly involved with the residents, I enjoy getting to visit and assist where I can, while overseeing financial matters.

Debbi Hamlet, RN, Director of Nursing
Debbi has 22 years of experience within Long-term Care. Debbi was brought up in Las Cruces, New Mexico. She lived in California's San Juaquin Valley for 18 years prior to settling in New London, Texas. Debbi received her RN ASN from Excelsior College in upstate New York, her LVN from Porterville Junior College in Porterville, CA, and her AS Business Administration degree from College of the Sequoias  in Visalia California. She enjoys reading and cooking as well as camping and fishing. Debbi has dedicated her career to the needs of the geriatric community ,and is proud to be a member of the Autumn Leaves Nursing and Rehab Team.

Jarae Standley, Staffing Coordinator
I began working at Autumn Leaves as soon as I graduated from nursing school and I am still here, that was years ago!  I began as a charge nurse, I am now the Staffing Coordinator, I love working with the certified nurse aides and medication aides.  I enjoy the residents and I have become very close with many of them and their family members. I would welcome the opportunity to provide you with a guided tour of our beautiful facility!

Natalie Elmore, HR Manager

I started working with our corporate office in October 2010 as the Administrative Assistant. Over the years, I have been driven to learn and do more. I am currently the Payroll, HR, and Accounts Payable Manager at our corporate office, serving our sister companies. I have also been given the opportunity to expand my role with our company by serving Autumn Leaves as their HR Manager. I love what I do and I strive to make the best of each and every day!

Dessie Benton, Dietary Supervisor  I received by Certification in 1991 in Tyler. Overall, I have 27 years experience as a Dietary Supervisor. I enjoy cooking for the Autumn Leaves residents, working with people, and Gospel.
